| Collection Notes: | Collection Kit with instructions will be provided by
physician or at an RRH Collection Station. Instructions: 1.) Fill out Personal Information on collection tube (Name, DOB, & Collection Date) 2.)Unscrew the purple cap making sure to preserve the liquid in the tube. 3.)Poke applicator into 6 different spots of the stool specimen. 4.)Screw the applicator back into the tube and secure tightly. 5.)Insert Collection tube into the specimen pouch and seal. 6.)Complete the return address label on the mailing pouch. 7.)Insert a copy of your order along with the specimen into the mailing pouch and seal. 8.)Return the mail pouch to the collection station (Preferred) or using the pre-addressed label - affix postage (Requires Additional Postage to mail) and mail back to the lab. |
