| Collection Notes: | Collection Instructions should be provided to patient by their
physician. Specimen Requirements: Patient should refrain from sexual intercourse or ejaculation for a minimum of 48 hours but no longer than 7 days before collection. THE SPECIMEN MUST BE COLLECTED BY MASTURBATION ONLY. Collect semen in a sterile plastic container provided by the laboratory or physician. Print time of collection on requisition and container and deliver to the laboratory promptly. |
